
PARTNERS IN CARE offers medical, emotional, and spiritual support to people with life-threatening
conditions as well as to their families, while together they face a serious illness. Our broad-based
support includes education, explanation about treatments, information about symptom management,
supportive counseling, referrals to community services, and assistance with planning.
Our staff includes a registered nurse, social worker and a chaplain/counselor. Our medical director is
a geriatrician who specializes in the needs of patients coping with life-threatening illness. Together
we work with the client, family, and health care team to create a plan that suits the unique needs of
the client and his/her support persons. We help patients and families identify treatment options and
manage symptoms. Through telephone calls and home visits, we are available to discuss the emotional,
social, spiritual, financial, and physical adjustments required by the illness. Partners in Care can
also help coordinate in-home care services if they are needed.
