Finances: Making arrangements so that someone can pay bills and take care of business is important

Health Care: Making clear what your wishes for health care are when you are not able to express those
yourself can be a tremendous help and relief to your loved ones. Filling out an Advance Directive
or POLST form will allow your wishes to be honored. These forms
can also be obtained through most health care organizations, which can also answer your questions about
them and help you fill them out.
Quality of life means different things to different people. Deciding what gives meaning to your life is
important, and understanding how various illnesses and treatments may impact this can make decisions
about your health care clearer and easier for you and your family.
Starting the conversation: It is never too early to start making these arrangements and it is important
to have these conversations with your loved ones and doctor. If something unexpected happens, then you
have the necessary papers in place. If you have a serious illness, having these things out of the way
can allow you to focus on getting better, and enjoying your life and your family.
